Regarding issue 2), this is most probably due to the cpu mode setting:
The nova.conf needs the setting "cpu_mode = none" in order to not have it set
to 'host-model'.
This setting has not been working on s390x - this is a statement as of Mitaka
and libvirt 1.2.21 at least

finally i find the cause of this issue.
The problem happens every time logrotate rotate the apache logs.
cat /etc/logrotate.d/apache2
/var/log/apache2/*.log {
weekly
missingok
rotate 52

delaycompress

create

finally i didn't deactivate the weekly crond tasks, I configured the weekly
crond tasks to be run at 05:47 AM every sunday.
Anyway, last sunday the apache2 process died again. This time at
05:15:13 AM.
Any update in this bug report?

thanks for you quickly support.
I can confirm the apache2 package version, it was the previous one,
2.2.22-1ubuntu1.2
We are using the apache2 prefork version, not the worker.
This issue just happens in one of our servers.

I just head-desked...
/tmp was mounted on overflow with a size of 1M. No wonder Downloads
don't work.
umount /tmp solved the issue...
Leaving this information here in case someone else fumbles in the same
way.

Sorry, I've been too busy / other problems since today.
I've upgraded the installation that was affected (3.1.19-1ubuntu3.12.04.1 has
been installed), removed the workaround in rc.conf, rebooted and works fine now!
