I'm sorry, but there is just not enough information to make any sort of
diagnosis on what caused the issue.
The 'grub-probe' error messages in the upgrade can be ignored. They're there
only because grub-pc package is installed, but it is not being used. The
pv-grub aki from amazon is

The system was built around 05/2011 and until 10/2011 updated at least
monthly, each update involving a rebuild of the AMI. I currently can't
see which one was the original AMI from Ubuntu.
The Region is eu-west-1a.
