I cannot reproduce your bug:

  * re-index - "su openldap slapindex"

This command doesn't work: the openldap account has a shell set to
/bin/false. Thus su openldap CMD doesn't work:

r...@t-slapd:~# su openldap " echo DDD"
r...@t-slapd:~#

Try sudo slapindex instead (and correct the ownership of the files in
/var/lib/ldap if necessary).
