This is an upstream bug that was solved in 5.2.13.
The Debian bug page has a patch for the 5.2.6 package, but I haven't
tested it myself.
This and other, less critical bugs solved in 5.2.13, so it may be a good
idea

Unfortunately I seem to be unable to get this bug triggered with the
reproducer. It could be a detail of the guest setup I am missing. Since I do
not have access to RHEL I used CentOS 6.3 in a 8core guest with 2 virtio disks.
Host was 14.04. Left the script running for quite a bit but no crash

If you're seeing problems on precise, could you file a separate bug
detailing your issues? The code path which this bug fixes doesn't exist
in precise, so you must be seeing another issue. :)

looks as through 4k disks are already working properly in Trusty.
@Mauricio, could you please confirm this? I've prepared a patched parted

If that's the case and the disk is already partitioned, I expect it to
work, as kpartx is patched and will use the right offsets for dmsetup.
If the disk is not partioned, thus partitioned w/ parted, then kpartx
