I believe this may be a kernel regression. I can repro on 15.04 with
sddm by installing any 4.x series kernel, haven't done any bisecting,
and can't find anything interesting in dmesg (when I can switch to a vc
sucessfully after X starts but fails to output anything), but with a

No it wasn't. I saw this issue in 13.10, 14.04, 14.10, 15.04, and now
15.10. So if it was fixed in 12.04, then it was only fixed in that version.
Funny that I have a new bug altogether and yet the symptoms are identical
in every way. More liked Occam's Razor states that either the issue was
never
right, pkg-xorg hates 3.0(quilt) :) (because of it's shortcomings), so
it's used directly if needed

The "Xlib: extension "GLX" missing on display ":0"" message suggests
it's indeed a problem with the GPU. I found that it could be caused by
bugs with nvidia drivers. This could be the case, as mine it's an
optimus laptop with NVIDIA GTX 850m.
