I've been observing this happen once a week across a wide range of x86
systems with USB keyboards in the last (5?) years.
Since I've observed it in the virtual terminal and have never observed
this with PS/2 keyboards, I suspect there is an event race condition in
the USB input stack, eg a
I have been experiencing this issue on 2.6.35-rc3 when a bunch of kernel
debug options are enabled - let me know if you'd like the .config (tuned
for an Intel desktop board). It reproduces reliably usually within an
hour of activity and we get a similar signature [1] to other reports.

This bug is disappointingly still present with the Catalyst 9.10 (8.661)

I'm still experiencing this with fglrx 8.66.10 (Sep 3 2009) / Catalyst
9.10 from the Ubuntu repos on 9.10 x86-64 with Radeon HD 3400 hardware
(in a Thinkpad T400), and I'd say this would be (currently) the biggest
drawback of the fglrx driver.
