I'm not sure if this is related, but I can still reproduce a GPU hang (which 
results in no graphics acceleration and a lot of intel_do_flush_locked failed: 
Input/Output error messages terminating programs) by running cairo-dock in 
direct OpenGL mode (indirect works fine...).
However, a lot of other things that would previously cause this crash (high GPU 
load, cairo-dock in indirect-rendering, Google Gravity demo running in Firefox, 
...) now work fine. :-)

Regarding the backends, I used gl/x11 for the last few days and did not 
encounter the lockup. While at the same time, the machine will lockup instantly 
(after 5secs) when using -vo xv. Sorry, that this does not confirm your 
suspicion but maybe my gl backend does not trigger the code path you mentioned.
Maybe unrelated but I noticed my system was running very hot but the CPU was 
idle and powertop also said the CPU was running the lowest frequency mode (800 
MHz), the system temperature almost reached 100 degrees celsius. After I 
rebooted the system now runs at about 40 degrees which I consider normal. 
Unfortunately the system is now unable to generate output via external display 
connector VGA1. I hope this is not a hardware damage and will try to generate 
output using a live CD and another computer. Also probably unrelated since 
13.04 I experience suspend/resume bugs, where the system crashes on resume. All 
this issues together are very annoying and I am considering downgrading as well.

Well, I rally tried to identify the cause of this problem but with no
success. There isn't a pattern. Anyway, I'm almost sure that is the
video driver because of the following: one thing I forgot to mention is
that, as a workaround, when the system is lagging I press the Super
button to open the dash and the system stop lagging. It seems that it is
a composite/driver/3D issue. Using other desktop environment like
Enlightenment (with composite enabled) does the same lagging - so it is
not related with unity.

About the command you said, could you explain me more about?

That command disables the kernel polling for new outputs, it should be
using hardware interrupts anyway.

Another thing you can try is whether change cpufreq scaling governor
makes a difference, in particular using performance. For instance, the
memory clocks are driven by the CPU clock so as cpufreq slows the CPU
down, the GPU becomes starved.

Craig: I'm curious, what apps are you seeing looking bad because of
this; my understanding from what Chris  is saying and from your xdpyinfo
is that it's just the default that's 16, so any app that wants to do
nice graphics should be able to ask for the highest quality visual
rather than the default.

Not only in the game (I'm playing Team Fortress 2), in the desktop also
very very laggy moving cursor when pressing keyboard. I don't know
exactly when this issue was appeared, but this happen after I'm changing
and updating my kernel, but when updating kernel also install other
update about X11. I don't know are this issue is affected by PAE or not
because I'm changing and updating my kernel into PAE.

I'm using Ubuntu 12.04.2 LTS with current kernel 3.2.0-48.
When playing TF2, replug the mouse can resolve the issue at a while, but after 
death, after not pressing keyboard and even still pressing keyboard, this is 
issue appearing again, and I'm replugging again, but at a while happened again 
and so on.

I have tried a new build of the savage package supposedly including your
changes (https://bugs.archlinux.org/task/34527), but it doesn't solve my
problem. I'm still getting my screen scrambled, just multi-color lines
everywhere. I have tried a configuration forcing to use EXA module as
well as a brand new xorg.conf file generated through X -configure
command.

Please, let me know if there anything I can you to help you find the
problem.

Regarding my last entry, fortunately my system now recovered from the 
behaviour of not detecting external monitors.
Concerning the hangup issue I want to add that HTML5/Flash videos also does not 
seem to trigger the hangup, however I am not aware of the subsystem used for 
this video playback (browser is Chromium).

 Check the 72%CPU usage above. It may have something to do with 
suspend/unsuspend back and forth.

After reboot system behaves fine, but slowly degrades with Xorg cosuming
more and and more CPU
