try to use an _untouched_ install media for source, just for a test!
if it still fails, maybe it is a problem of the server side (can be
network traffic problem, but not 100%). first you have to create a
_clean_ file storage dir for a test-os.

i saw this kind of problem, and finally the solution was to go back to
a clean install media. something, somehow had gone wrong. a test is
cheap :)
