Tere is something wrong with startup script under Ubuntu 14.04
1. There are 2 jsvc.exec processes running in memory
2. There is no cloudstack-management.pid file at /var/run
3. I can't stop process using /etc/init.d/cloudstack-management stop. It
reports OK, but processes are running.

You say you used 'pre-setup' but what type of storage is it, NFS, iSCSI, FC etc
It looks like the XenServer cannot write to the storage, so have you checked
permissions and also confirmed that the XenServer can create files directly on
the storage etc

I’m not sure, but I’ve hit such an issue when I removed the log files.
First kill all java processes (CloudStack related, if any). Can you try this:
chown cloud:cloud -R /var/log/cloudstack/management
And then try to start the management server again?

my guess is your are missing the vhd-util , please download and copy the
vhd-util to the xenservers.
# find / -name vhd-util
/opt/xensource/bin/vhd-util
/opt/cloud/bin/vhd-util
/usr/bin/vhd-util

I have fixed the problem by re-installing Ubuntu server OS from ISO, downloaded
from the official site.
The former Ubuntu server has been installed using PV guest settings from here:

When the deployment goes through, the allocation placement algorithm
will tell you if cpu over provisioning worked. You can see it in
management-server.log file..
Here is an easy way to extract useful info for failed vm deployment.
