authrequiredpam_tally2.so onerr=fail audit silent
deny=5 unlock_time=900
Anyone have any idea why this was interfering?

Still stuck, but here’s the output of the command with --debug turned on:

Thanks for the response, Ken. I thought that might be the case, so I tried it
with selinux disabled (setenforce=0). Same exact error. :-/
