I'm finding "phantom" alphabetical index entries in some Writer documents that I can't figure out how to get rid of. After creating a new alpha index in a document, I'll find a word included in the index that doesn't exist in the document. For example, I might see the word "elephant" in the index, but if I search the document for "elephant", the word does not appear in the document. I've gone so far as to open the document as a zip file and look in the "content.xml" file. There I find entries with "elephant." I don't know where they came form or how to get rid of them. Any ideas?

BTW, my computer is a Macbook running Lion.
