I had this same problem. I wound up copying my OO spreadsheet information into
Apple Numbers then exporting those addresses to a csv file and creating the
labels using Apple pages merge file. See the web address here for a little

Are you using the official way to create labels (File | New > |
Labels)? If so, create your page image - with the field names you
require - but don't select the required records
