I updated to the latest build of LO 3.5 and tried using a different name.
It worked.
I also tried the same with Write2epub and it worked, but the formatting was
horrendous.
The plot thickens.

Don't know how that works on Mac exactly, but how about trying to create
a pdf with a different name?

There is no need to look for a trace program.
If you go into Utilities > Console you can find a log of what happened.
All the actions are timed.
I have taken on something rather demanding at the moment, but I will try to
do the Export PDF again when I find time.

File → Export as PDF and in "General" section (under "General" tab) there is
tickbox for "Create PDF Form".
