I already had this kind of error, not with cyrus but dovecot.
Either a cyrus misconfiguration or authentication with sasl (?).
Have you checked your sasl config?
testsaslauthd -u /user /-p/password/
Have you got cyrus logs?

issue with saslauthd :
# testsaslauthd -u olg -p MyPasswd
0: NO authentication failed
Here is my configuration :
# more /etc/saslauthd.conf
LDAP_SERVERS: ldap://localhost
LDAP_DEFAULT_DOMAIN: percy-test.com
