You could make a modified smtpd server only for autenticated users (it's
only another "tcpserver qmail-smtpd-mod" on another address. On this
tcpserver server you'll not enable rbl checking.
In this way you achieve easily your goal and make distinct smtp server for

No, because rblsmtpd looks up the IP before SMTP-AUTH is (or can be)
done.
