I have this configuration

If you're running the latest 0.42 patch from the site you mention 
above, then you shouldn't have the hostname in the run file.  It should 
just be .../var/qmail/bin/qmail-smtpd /home/vpopmail/bin/vchkpw 
/bin/true 21.

We will probably need to mention this in the upgrade documentation, and 
it could be a messy install for some people unless we can come up with 
an update patch for qmail to go from the 0.3.1 auth patch (which is 
what many people are using, I believe) to 0.4.2.
