Perhaps the list is a subscriber of itself, resulting in a mail loop?  
What do the headers look like in one of the later copies of the 
message?  What does /var/log/qmail/current show -- any odd error 
messages?

If the list is in a .qmail-name file and that file has an alias (path to
Maildir) line in it, and that use does not exist, then qmail will keep
deliveriing to the list, deferring on the non existant user and 
re-attempting
after a timeout period. That is one reason why qmailadmin was switched
to use forwards instead of aliases.

If this is the case, then change all the lines to be forwards of the form:
[EMAIL PROTECTED]
[EMAIL PROTECTED]
etc...
