I've found that Patch PHSS_38140 causes the same problems for HPUX
11.23 and Veritas Netbackup 6.5.x.
It was part of the September 2008 QPK bundles, and Veritas stopped
working.
We've removed the patch, and Netbackup works again.

When we upgraded on HP-UX, we had a couple bomb outs during the process
as some files that were slated to be upgraded were still being used...
Be sure that all NB processes on the master are stopped prior to the
upgrade...
