I recently downloaded echofon for free and it is now my main twitter client. It
is pretty voice over friendly in my experience.
Here's a short review i wrote for it, comparing it to the official twitter app
and tweetcaster.
Echofon works well with voice over in my testing.

I have been using this ap for almost 2 years. And it sends you all
notifications, mentions, dm's, and retweets. I also love how it handles
multiple accounts

settings I asume, I did it so long ago that I can't really remember. I know
you have to go to accouns, then settings, after that I can't remember
