If you use Fantastical II, then you can have both reminders and
calendar appointments in the same place. Also, Fantastical lets you
set more than 2 alerts before a calendar event. At times, I have set 3
alerts for a doctor appointment, 1 a week before, and the other two

The biggest difference in the Calendar app and the Reminders app is that once
the time of an event in the Calendar goes by, you will not see it again. But
in the reminders app, each reminder has a checkbox, and it will remain in your
reminder list
If I want a reminder a day before an appointment, this is what I do: I say,
"Set a reminder for April 10" Siri will ask, "what do you wwant to be
reminded about?". Then I say, "I have a doctor appointment tomorrow at 11:00
