Possibly you are holding the phone such that you are covering the microphone at the bottom near the lightning plug.
Possibly your case is interfering with the microphone. Possibly your not speaking loudly or clearly enough. Try holding the phone up so the lightning port end is within 6 inches of your mouth or so and make sure you hold it by the sides.
