Do you have audio ducking disabled? If you turn off audio ducking then Voiceover becomes very quiet during a phone call. If not, then it's a matter of placing Volume into the rotor. Once you started a phone call turn your rotor to volume and flick up with one finger to increase Voiceover volume. You can then also reduce the call volume a bit by using the volume down button and hopefully achieve a good balance where you can hear Voiceover as well as your call.
