It seems to happen when iTunes match is on. Current work around is to turn off
the music app in the app switcher. Every since I have done this, the problem
has gone away.

music if calls are received. This is true as well if the screen is locked
for my Four S.

It happens on occasion for me, but it is definitely not consistant at least
