I had the same problem, did what you did, and the wild punctuation persisted.
Days later and a replacement 6s phone, and the problem was gone.

I am running the latest commercial iOS on my iPhone XS Max. Punctuation has
gone absolutely wild, mostly in the mail app, but also another apps in which I
want to read text. I have consistently gone back and change punctuation level
to none, but voiceover is reading every punctuation mark.
