I too have this issue and while not an ideal solution I turn Voiceover’s volume
down when I only require the time.

I have the same problem. When I am working, I want to find out the time, and
sometimes it will just not vibrate. Other times, it works perfectly. I have
read that others seem to have the same problem. If anyone has the solution, I
would love to know what it is.
