Actually, you ask an excellent question and one that I am most delighted to 
answer.

First, I display my caller ID to everyone in my Contacts list.

Second, I find that I do not necessarily want to display my caller ID when 
responding to advertisements such as billboards solicitations or unverified 
business listings.  Does this make sense?  

I'll give you an example:

The other day, I passed a house that is for sale that listed the real estate 
agent as a point of contact.  I did not want to display my caller ID to the 
agent until I actually spoke to the agent.  

Many times, at least here in the US, calling a business with your display 
showing can add you to a robo-call list that is most annoying.  I don't have 
robo-call problems because, I do not display my caller ID to unknown/unverified 
people/businesses.

On the reverse side of the proverbial coin, I am fortunate enough to have my 
eye doctor's personal cell phone number.  I assure you that I do not abuse the 
privilege (smile).  However, when he calls me from his cell, his caller ID is 
not displayed because he does not want people to have his personal number 
unless he gives it to them.  

Yes, yes, he could use a second phone for his outgoing calls but he does not 
and his carrier does not allow for the kind of calling code described in my 
initial post to this thread.

Remember, there is money in data these days so, very often, advertisers want to 
entice callbacks not just for the stated advertised product but also in order 
to capture telephone numbers for future robo-calls, surveys, etc.  

I'm one of these radical thinkers who believe that calling my local hardware 
store should not result in me having to display my personal data.  After all, 
the owner of the store is not displaying his/her personal phone number to me, 
right?  (smile) 

Finally, remember that one's caller ID is always displayed/available to the 
person/business who owns a toll-free number.  This is because the owner of the 
number is paying for the call.

I'm not paranoid about displaying my caller ID but I am protective of my cell 
number.  I suppose, one day, sooner or later, if Apple does not implement 
selective caller ID display, I will lose the functionality I now enjoy but, 
until that time, ...

asking because I do not like to answer calls unless I know who they are from.  
What do you have to hide that you would want to call someone and not let them 
know who is calling? Just curious.

of the reason why I have stayed with them so long.  

I just don't understand why Apple will not allow selective hiding of caller ID 
as it was back in the days of Windows Mobile 5.x and so on.
