Yes, doing it via the phone is also a good option.
We live in a small town and I know the pharmacist at one of our local 
pharmacies so I can just call them and make an appointment and then they still 
put it in the system of course so my record is updated accordingly.

I just had a look and this is entirely stupid because as soon as you double tap 
on the date of birth a date selector opens, but instead of the usual 3 boxes 
where you can select the year, then the month and then the day, it comes up 
with the current month and year, e.g. October 2022 and you then have a calendar 
where you can tap on any day in October 2022 and it would be selected. There is 
also a picker box where you can flick down and it will change from October 2022 
to September 2022, then to August 2022, then to July and so on. Of course this 
is ridiculous because I tried a double tap and hold then dragging the finger 
down the screen to make this go faster, but it doesn't work so you literally 
would have to flick down hundreds of times to get to your month and year. I 
believe you are somewhere around my age, can't remember, but I was born in 
April of 1967 so I would first have to flick 9 times to get from October 2022 
to January 2022, then for each consequent year I would have to flick 12 times 
so in order to get from January 2022 to January 1968 (54 years) I would have to 
flick 648 times + the 9 flicks for this year and then I would still have to 
flick 9 more times to get from January 1968 to April 1967. So, after 666 flicks 
I could then find April 19 and double tap on that and you better make sure when 
you try to flick really fast you don't mess up because that what happened to me 
when I was somewhere around 1980 and the picker closed and when I opened it 
again I was back in October 2022.
As I said, this is ridiculous and I am pretty sure I would have found a 
different way if there was one so this is one for the computer for sure, when I 
just tried this on my PC the birth date was a simple edit field and it says you 
have to enter your birthdate in the format MMM DD,  so this means you write 
the first 3 letters of the month like Jan, Feb, for April it would be Apr or 
for August it would be Aug, then  you write the day like 02 or 27, then put a 
comma, then put the 4-digit year like 1967 or 1972 or 1959.
There is also a date picker on the computer where people who work in a visual 
mode can easily pick the date and I assume a sighted person can simply drag 
their finger down on the picker in iOS, but it doesn't work with Voiceover.
One last comment, I went back to my iPhone because I initially tried this with 
Safari assuming this is what you use. I use Edge as my default browser on the 
iPhone and while Safari may have this feature and I don't know about it, in 
Edge you can open a website, then tap on the "Browser" tab at the bottom and 
select to "Open as desktop site". This means it will open the desktop version 
of the website and not the mobile version which would be the default on an 
iPhone. When I did this I was also able to enter my birth date on the iPhone in 
that simple text format I described above.
