Does VO say speech off, before going quiet?
I am thinking you might accidentally be touching with three fingers.
If not, is this one specific app, or all apps?

Yes, had the same issue starting with 13.0.1.
Solution: one finger double tap right on the "Diction" button to start diction
and repeat to stop.
