3 buttons: +, -, and a blank round one, which should be the one you want.

Pressing the round button should answer, pressing it again should hang up.
This is the way it worked on mine, from 2012, they may have changed it.

I did hear on a podcast Today In iOS that some people are having problems with
bluetooth making calls in there cars. Apparently thee may be a problem in IOS
8.0.2. Can anybody confirm this?

It doesn't have to do with being signed in with facetime per se if you are
using handoff; at least I don't think so. It has to do with being signed into
the same icloud account with both devices and being on the same wifi network.
