I figured out what I did to cause my iPhone 5 to speak the caller's name when talking with someone using my earpods. I changed a setting in VoiceOver to turn on Speak Notifications in Locked Screen. Since the calls where I was having the problem were taken while walking with my phone in locked mode, the phone was notifying me of the caller's name based on that setting. I know better than to change the default setting which is off for this item but as I say, live and learn.
To get to this setting, select Settings, General, Accessibility, VoiceOver, and it is the last setting in that section.
