I've not had any trouble with the two-finger double tap playing whatever the
last audio content was, whether that be music, radio

Even with VD not running, the 2 finger double tap, sets it
off. Doesn't happen on my Iphone, just this IPad mini.
Strange.
Maybe a re-install of all apps?

when I double tap say on the youtube player when listening to an
audio stream, or playing a podcast, Voice dream starts reading the
last book I was reading.
In other words, it appears to have captured the double tap keystroke
which used to start and stop audio playback.
How can I fix

let's say I've started an audio stream. I do a double 2 finger tap
to pause the stream, and voice dream kicks in.

Well, I started a podcast playing, paused it with the two-finger double tap. I
then started a youtube video playing, and paused it with a two-finger double
tap and the podcast began playing.
So, I think the problem app is Youtube. It isn't picking up the two finger
double tap, or podcast,
Just remove Voice Dream from the app switcher and that should correct the
problem.

Suggest you delete VD from task switcher. So it's not even a background
process.
