This is a wiki farm configuration. Issue has been present since MediaWiki 1.21.
Uses GlobalSettings.php available in the same server as other wikis, but not
accessible in any way via the web.

Are the permissions assigned? Is the only problem the checkboxes?

According to the database, the person has user rights, it's just there are no
checkboxes and that the person isn't really an admin; he does not have access
to admin or bureaucrat.
