This seems to be related to the DuplicateReferences gadget. "wikibase-referenceview-heading" doesn't exist in read mode, the gadget insert this one. The "delete reference" isn't hidden after saving, causing it to stay

We're still getting quite a few duplicates. In the 2016-02-15 dump I found 951 sitelinks that appear more than once, in the 2016-10-10 dump there are 3914. I haven't checked all of them, but I've already come across a bunch of examples with quite a bit of time between the

I conclude that we should run one query per term type here and simply merge the result in the application code.
Yes, this is a property of Btree indexes- 2 ranges cannot be handled efficiently- you can only handle one with an index at a time. You could even merge both

Title is already a first-class entity, that's what the page table is.
No, right now, page is an entity that has a series of properties: at title, a text, etc. By setting title as a strong entity, it has meaning by its own: a page has 1:1 titles, a title has 1:0 pages.
