copperplate/heapobj: sanitize shared heap management

Simplify heap management when --enable-pshared is in effect:

1. create a separate backing store file in tmpfs only for the main
heap, for sharing between processes in the same session. All other
shared heaps are nested in the main heap.

2. Get rid of *_shared() call forms from the API, assuming that
heapobj_init() is implicitly asking for shared memory when
--enable-pshared is in effect. Only *_private() call forms are
required to force a private heap allocation, even when
--enable-pshared is set.

3. Introduce __heapobj_init() and __heap_init_private() for accepting
a user-defined heap memory area (when applicable. Conversely, regular
heapobj_init() and heap_init_private() calls allocate the storage area
internally.

4. Introduce heapobj_bind_session() for obtaining a mapping on the
main heap of a given session.
