I translated and uploaded a (empty) version of the form here: http://perso.3sr-grenoble.fr/users/bchareyre/nolist/nolist_yade/OpenScienceResearchSoftwareAward.pdf They make it clear in the preamble that they want facts and figures, not long texts. And it is true that you probably gathered similar pieces of information for the dutch thing. So, if you have pieces that fit in the boxes it can help (I could also go back to your documents...).
