'Configuration' widget with a * so that a user would have to fill this
form out and enter a metadata customization script

Nova handles new feature request through launchpad blueprints. Please
follow the process described in[1], e.g. open a blueprint in launchpad
and push a specification according the the spec template to the
openstack/nova-specs repository.
I feel this is part of a bigger development effort (supporting usb for
nova instances?). Please propose a blueprint / spec for your feature
first where we can discuss the general idea of your feature. Then when
you need actual development help please propose the actual coded, where
you got stuck,
