I got it :

<dtml-with "mydoc">
    <dtml-var render>
</dtml-with>

rather than :
<dtml-with mydoc>
    <dtml-var render>
</dtml-with>

after all, this is quite logical ;)

ps : The idea came to me after playing with :
<dtml-var "mydoc.title_or_id()">
