So then how can ssh-agent be run, only once, automatically at the command line login? I've heard of people using special PAM modules to perform this, but I can't find any docs on it.

that's a bad idea, since many programs use
$SHELL (pw->pw_shell)
to execute programms.
additionaly, for every loginshell a new agent gets
started and agentforwarding will break.
moreover, you need a _single_ program as a login shell.
