The common usernames are not an issue.  I have the same username on
client and server most of the time.

Based on your description, it sounds like you've ONLY tested
user_A -> user_B, and user_A -> user_A.  So the point of failure is not
"user names are the same", but rather, "user_A on the server".  Which
leads me to suspect you have incorrect permissions (or an invalid key)
in user_A's home directory.
