[SaF] Personal Report Format

Hello all,

I now prepare to write my personal performance evaluation 
report/yearly, since i'm a fresh to work, i really have no idea as to
what i should to begin.(not because i'm lazy) so everybody knows where i
can find some resource(format)on the Internet, i know some people are
good at writing such report. i'm now working in puchasing & logistics
field.
