The latter. I am going to put an idea in your head just now.

exceptions to that, but I could not find anything like that on the web.
Mexico has a National Health plan for it's citizens and it just does
not meet the kinds of needs your sister's daughter would require. The
richer Mexican citizens and Americans living in that country, usually
go outside the National Health plan and pay more for private doctors or
go across the border to American doctors and hospitals.
While I am hesitate to agree with your sister's plans, I will offer you
this suggestion -- look to the Catholic Church and other churches that
do charity work in Mexico. Here is a site listing many such orders
