That is ominous... I should have also suggested adding "&debug=true" to the
URL, which might make it clearer where the error is coming from.
If that doesn't help, my usual checklist is to try switching to Vector (if
you're using a custom skin), and temporarily disable all extensions except
for Semantic MediaWiki and Semantic Forms, to see if the problem is coming
from the skin or another extension.
