i guess he wants to have only one inbound/outbound gate and secure
everything else. (This is also my mission..)
So the sbc is the only instance exposed to the net, the registrar would
be located in the internal network. I know it was mentioned before,
where to do the nat-handling and so on, but also it would be nice and
also commercial SBCs support this scenario already.

application to handle the REGISTER request (this is what shows up in
the logs). Is there any way to have these requests forwarded directly
to the SIP server specified by the device? Or is there some other way
