propagate app_name to everywhere it is needed.
shouldn't we also possibly figure out the application parameters in
the core and pass them e.g. as map<string, string> to the app? That
way they can be properly combined and/or configured (P-App-Param
header or R-URI params, statically configured and/or passed via DI/RPC
call). Some of that code in DSM module could be used, and that one
simplified, too.
