Good Afternoon-

We are just in the beginning stages of rollout and implementation for early 
identification/screening and treatment of severe sepsis at our hospital. 
Additionally, we are also participating in a regional initiative with 9 other 
hospitals, some of which are 'critical access' hospitals only. These hospitals 
are starting some of the EGDT and then shipping the patients to us. My 
questions are the following:


1.       Does anyone have some type of a transfer checklist that the transfer 
coordinators use when either sending or receiving a septic patient from an 
outlying facility to show what has been done and what symptoms to ask about?

2.       Has anyone involved EMS? And if so in what way? Does EMS do any type 
of screening or have any types of protocols in place for when they respond to 
this type of a call or transport this type of a patient?

3.       Lastly, does any anyone have a hand-off report form that they use 
either paper or electronic between units ER-ICU/floor,  Floor -ICU, ICU-OR,etc.
