We use >4 (arterial) for EGDT. For >2 we still do early cultures, early antibiotics, 20-30ml/kg fluids over 1-2 hours, glucose control (<180) only if they are in ICU, vent plateau pressures <30. Basically everything except those goals requiring a central line, since they are not required to get one for this lactic acid value (>2 <4.1), unless the SBP <90 or MAP <65 despite adequate fluid resuscitation.
