Hi Doug,

High lactate exists in the subset of patients with sepsis who have
established oxygen supply/ demand imbalance, so it would never be used to
confirm the presence of sepsis, merely to assess severity and guide
resuscitative attempts. It's not in the slightest specific to sepsis.

That said, it may help identify patients with hypoperfusion or shock prior
to other clinical signs, so it does have utility at the bedside.

We're using POC lactate in several of our Ambulance Services, and we intend
to explore whether or not high PH lactate is predictive of outcome.
