Good afternoon;
 
The EMS proposal was for Point of Care Lactates be sampled, like in Finger 
stick Blood Glucose sampling.  We had found recommendations and methods to 
support this, but then discovered the only FDA approved POC Lactate meter had 
been discontinued.  
 
Our research buffers your statements that EMS blood draws for lactate is not 
supported.
 
Wondered if anyone had encountered these roadblocks and any suggestions for 
working around them.

Lactates need to be drawn and put on ice. Drawing in the field does not provide 
you with an accurate result. We draw the lactate in triage.

We currently do not have our EMS do a lactate level in the field either.  
Recently we have been noticing it seems we are having hyperelevated lacte 
levels in our
 emergency department.  One thing to think about – if the EMS brings in a 
patient they may be bringing in blood as well and if we draw a lactate on this 
blood – if it has been drawn some time ago may give us false high levels 
correct??  Anyone having this issue
 as well?

Working on developing a prehospital EMS based screening system for sepsis and 
severe sepsis, found out that the only  Point of Care Lactate meter FDA 
approved and CLIA waived has been discontinued on the market and in production. 
 There is currently no other
 FDA Approved/ClIA waived POC Lactate meters.

 

Has anyone else ran into this issue or problem solved it?  Working in NY and 
with our SEMAC and REMAC's we would need approval to use non FDA approved or 
CLIA Waived devices. 


 

Current plan is to use all of the sepsis screening criteria minus POC lactate 
and educate implement without .

 

Any thoughts, suggestions, or solutions from the group is appreciated.
