Hello, My name is Luis Figueroa. Currently, Im an Intern (PGY-1) in an Internal
Medicine Residency Program in Puerto Rico.

I was recently reading the new guidelines of the SSC and I come a cross with
the great idea to apply the bundles and follow the recommendations of the
guidelines in my institution considering that we have a good incidence of
patients admitted with criteria the severe sepsis and/or septic shock to MICU
and IM ward floor.

I am really motivated to conduct a cohort/prospective study creating a
checklist of the bundles to each patient with criteria of severe sepsis and do
all the respective approaches such as measure lactate levels, IVF .9 30ml/kg
etc, and monitor these patients for lactate clearance and procalcitonin levels
related with Abx deescalation. And then look back retrospectively and analyze
in record room charts of severe sepsis that did not follow the bundles and see
the impact in mortality benefit!

I proposed this idea to the chief resident, and told me "the idea is great,
make a proposal and checklist and I will speak with an attending in critical
care medicine to be your advisor."

I would like to make a great impression to the advisor at the time of the
presentation the proposal!

Do you have any advice for me? Is there any member in this group doing
something similar in any institution?

I would deeply appreciate your time and support!

Regards,

Luis
