There was a validated prehospital sepsis detection protocol published in
the Journal of Emergency Medicine earlier this year (
http://www.jem-journal.com/article/PIIS0736467912014412/abstract?rss=yes).
Note that this tool depends on point-of-care lactate measurement. I had
been under the impression that this was the only validated tool in the
literature, but the European Journal of Emergency Medicine just came out
with a study a few days ago covering two older tools. I haven't gotten to
read that study yet, but the abstract is here:

I don't know of any full-fledged best practice guides for prehospital
sepsis, and if you come across one I'd appreciate it if you let me know.
Dalhousie University in Halifax, NS does have a brief evidence-based review
of sepsis interventions available at

There's also evidence to show that when prehospital providers recognize
sepsis the time-to-EGDT is significantly reduced.
